Cold storage failure spoilage program instructions
If the Immunomedics medicine prescribed for a labeled indication was spoiled due to a catastrophic event or redundant system failure and no portion of the product was administered, the product may be eligible for replacement through the Immunomedics Spoilage Replacement Program.
*Product must be returned or a Certificate of Destruction must be provided to be eligible for replacement.

Requesting replacement product
- Complete the Spoilage Replacement Form. Please write legibly and fill in all required fields. A Health Care Provider signature is required on the form.
- Fax the form to (866) 223-9537 within 30 days of spoilage occurrence.
- The request may take up to 5 business days to review. If approved, further instructions for returning the spoiled product or completing a Certificate of Destruction will be provided.

- Returned product must be received by Immunomedics within 30 days of approval.
- Replacement product generally ships within 10 business days following receipt of returned product.

- Each instance of spoilage replacement requires completion of this form. Replacement is on a case-by-case basis at the sole discretion of Immunomedics.
- Please retain all original product and its packaging for returns processing.
- Immunomedics does not ship replacement product if the spoiled product was prepared for an off-label indication. Immunomedics does not ship replacement product if ANY portion of the product has been administered. Immunomedics can ship replacement product only to licensed facilities.
- All spoilage requests are subject to review.
- Immunomedics monitors this program for trends and excessive use and reserves the right to deny product replacement requests.
- Immunomedics has an appeals process for denied requests. Please contact Immunomedics Customer Service to submit an appeal.
- Approval of spoilage replacement request is subject to certain limitations and conditions set forth by Immunomedics.
